We support you — every step of the way.
Everyone is affected by cancer in some way, and support beyond medical treatment is an essential part of care. At Community Care, we offer a range of services to help patients and loved ones navigate the physical, emotional, and practical challenges of a cancer diagnosis, treatment, and recovery.
Community Programs
Our cancer specialists are engaged in community programs designed to help you get the support you need during and after treatment. These include:
- Educational lectures and events to help patients stay informed about treatment options and survivorship.
- Tumor boards where physicians coordinate care to ensure comprehensive, multidisciplinary decision-making.
- Collaboration with community organizations to connect patients with additional resources and transportation assistance.
Our team is here to help you and your family feel supported, informed, and connected.
Nutrition Support
Proper nutrition is especially important during cancer treatment. We offer nutritional counseling with trained specialists who will work with you to build a personalized plan that fits your lifestyle and treatment needs. For more information, call Image Guided Radiation Therapy at (518) 213-0305.
Patient Navigators at CCP
When you’re facing cancer — from screening and diagnosis to treatment and follow-up care — you don’t have to go it alone. We are pleased to offer certified patient navigators who help patients and families make sense of the cancer care journey and connect them with services and resources that support their needs.
Patient navigators can help with:
- Coordination of care across screening, diagnosis, treatment, and follow-up
- Scheduling and communication with specialists and care teams
- Education and emotional support for patients and families
- Connection to community resources and programs offered through partners like the American Cancer Society and other agencies
- Resource guidance for financial assistance, transportation, home health care, support groups, and more
Our navigators serve as a single point of contact throughout many stages of care, helping to reduce stress and empower you to focus on your health and well-being.
Why Support Services Matter
Support services — including navigation, nutrition counseling, and community programs — are about more than logistics. They help improve care coordination, ease the emotional burden of cancer, and connect you with tools and networks that strengthen your resilience and quality of life through every phase of your journey.
